29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. A. Metalguard of Texas worksite located at 30354 Tracy Rd., Walbridge, Ohio: On or about October 7, 2014, the employer did not ensure employees were protected from fall hazards while replacing steel decking and exposed to a fall distance of 17.00 feet. An employee sustained serious injuries after falling through the roof insulation after the steel deck was removed.

29 CFR 1926.761(b): The employer did not provide a training program for all employees exposed to fall hazards: A. Metalguard of Texas worksite located at 30354 Tracy Rd., Walbridge, Ohio: On or about October 7, 2014, the employer did not ensure employees had been trained in the recognition of fall hazards commonly created during steel erection activities. Employees unfamiliar with the requirements of fall protection during steel erection activities were exposed to falls of heights at or above 16.00 feet.
