29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 7 , 2014, 32153 Mustang Hill, Bulverde, Texas , the employer failed to ensure that employee(s) were not exposed to a fall hazard. Employee(s) was building the roof trusses and was exposed to a twenty-two (22) foot fall hazard.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:   On or about October 7, 2014, 32153 Mustang Hill, Bulverde, TX, the employer failed to ensure employee(s) were protected from a fall hazard to the ground below. There was no guardrail system installed on the second floor edge openings were the future stairs would be installed. Employee(s) were exposed to a fall hazard of approximately ten feet and six inches to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  On or about October 7, 2014, 32153 Mustang Hill, Bulverde, Texas, the employees were exposed to a ten feet and six inch fall hazard. Employees were observed stepping on the top step of the portable, free standing ladder. The Werner ladder was used in a manner that is prohibited by the manufacturer.
