29 CFR 1917.44(n): Safety devices were removed and/or made inoperative on vehicles used to transport employees within the terminal.    Terminal 6:  On or about October 8, 2014 and times prior thereto, longshoreman were tying the seat belts in knots making them inoperative.  This exposed employees to potential struck-by hazards from not wearing a seatbelt while being transported off the vessel to the marine terminal.

29 CFR 1918.86(n): Drivers drove vehicles, either forward or backward, where personnel were in positions where they could have been struck.  M/V Kaijin:  On or about October 8, 2014 and times prior thereto, employees would be in between a parked vehicle and a vehicle being parked during loading operations.  This exposed employees to a struck-by hazard potential from positioning themselves in between two vehicles.
