29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:     On or about October 6, 2014, and at times prior there to, employees performed a cutting operation on a tank trailer where flammable vapors were present.  Employees were exposed to struck-by, fire, and explosion hazards when ignition sources were not controlled or eliminated.

29 CFR 1910.252(a)(2)(vi)(C): Cutting or welding was permitted in the presence of explosive atmospheres (mixture of flammable gases, vapors, liquids, or dusts with air), or explosive atmospheres that could develop inside uncleaned or improperly prepared tanks or equipment which had previously contained such materials:      On or about October 6, 2014, and at times prior there to, employees were cutting on a tank trailer that was unclean and had previously contained flammable liquids.  The employees were exposed to struck-by, fire, and explosion hazards.
