OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Well site: On or about October 30, 2014, and times prior to, employees were operating a service rig during the removal of a gas/oil well's production string with one employee working from the derrick mounted racking board. The derrick emergency escape system (the Geronimo) ground anchorage was insufficient. This exposed the employee to fall hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: Well site: On or about October 30, 2014, and times prior to, employees were operating a service rig during the removal of a gas/oil well down hole electric pump. The rig racking board guy lines were attached to inadequate ground anchorages and three of the rig guy lines ground terminations did not have the required number of wire rope clips. This exposed the employees to struck by hazards.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Service rig floor: On or about October 30, 2014, and times prior to, employees and another contractor's employee engaged in the removal of a down hole electric pump with a service rig worked on the rig's unguarded floor. This exposed the employees to the hazard of falling over four feet.
