29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous   558 Maple Street, Wethersfield, CT 06109 on or about 10/07/2014: A yellow extension cord was plugged to a wall outlet and it was missing the grounding pin. The wall outlet was not GFCI protected nor was it connected to a GFCI branch circuit.

The employee was exposed to the fall hazard.
