29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      Employees performing work at elevations noted below were exposed to the hazard of falling and the employer did not ensure open-sided floors and platforms four feet or more above adjacent floor or ground level were guarded with standard railing (or equivalent).      A) Employees performing trailer washing were allowed to stand at and pass by the access opening on the second deck of a two deck trailer and the employees were exposed to an approximate fall of 8 feet.      B) Employees performing service to the agitator motor and trailer wash water pump in the separator building were exposed to an approximate fall of 12 feet into a cistern containing a mechanical agitator and a water pump inlet in that an approximately 25 inch wide opening was present between motorized equipment at the edge of the cistern.

An employee was exposed to continuous and intermittent noise at 129.3% of the action level or an equivalent sound level of approximately 91.8 dBA during the 388 minute sample period.      An employee was exposed to continuous and intermittent noise at 148.9% of the action level or an equivalent sound level of approximately 92.9 dBA during the 349 minute sample period.
