Employee working in the production area and operating tube filler #5 was exposed to continuous noise levels of 88.65 dBA for an 8-hour TWA,  which is equivalent to dose of  83.01%.  This level exceeded OSHA Action Level of 85 dBA. Sampling time was 448 miutes.  Zero exposures were assumed for all unsampled time periods.    Condition observed on or about October 16, 2014.
