29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  (a) At Lake Taneycomo Woods water tower at Well House #1 on or about October 22, 2014 signs were not posted to warn of the confined space hazard.  There were no warning signs on the man-way cover to the water tower.   Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Lake Taneycomo Woods on or about October 22, 2014 had employees handling 1 gallon containers of chlorine at Well House #1 and did not have a shower and eye wash which could provide tepid water for drenching of eyes and body in the immediate area.  (b) Lake Taneycomo Woods on or about October 22, 2014 had employees handling 1 gallon containers of chlorine at Well House #2 and did not have a shower and eye wash which could provide tepid water for drenching of eyes and body in the immediate area.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. 29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:  (a) At Lake Taneycomo Woods Development on or about October 22, 2014 the wiring at the Well House #1 and water tower facility was not free from recognized hazards that were likely to cause death or serious physical harm in that wiring was not suitable for outdoor use and use in wet environments.  (b) At Lake Taneycomo Woods Development on or about October 22, 2014 the wiring at the Well House #1 and water tower facility was not free from recognized hazards that were likely to cause death or serious physical harm in that wiring boxes were not properly wired and provided with approved grounding.  (c) At Lake Taneycomo Woods Development on or about October 22, 2014 the wiring at the Well House #1 and water tower facility was not free from recognized hazards that were likely to cause death or serious physical harm in that the bus bars were not properly wired.  (d) At Lake Taneycomo Woods Development on or about October 22, 2014 a single pole switch was lying in a metal cabinet and exposed to the weather without being mounted in a box and provided with a weather resistant cover.  Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. A water faucet and water heater were located next to the cabinet.
