NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. (b) Lake Taneycomo Woods on or about October 22, 2014 had employees handling 1 gallon containers of chlorine at Well House #2 and did not have a shower and eye wash which could provide tepid water for drenching of eyes and body in the immediate area. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose. (b) At Lake Taneycomo Woods Development on or about October 22, 2014 the wiring at the Well House #1 and water tower facility was not free from recognized hazards that were likely to cause death or serious physical harm in that wiring boxes were not properly wired and provided with approved grounding. (c) At Lake Taneycomo Woods Development on or about October 22, 2014 the wiring at the Well House #1 and water tower facility was not free from recognized hazards that were likely to cause death or serious physical harm in that the bus bars were not properly wired. (d) At Lake Taneycomo Woods Development on or about October 22, 2014 a single pole switch was lying in a metal cabinet and exposed to the weather without being mounted in a box and provided with a weather resistant cover. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
