29 CFR 1917.112(b)(1): Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water:    (a) On the down river side of the permanently moored work barge, railings were not provided to protect employees from falling more than 4 feet into the water.    (b) At the barge end of the walkway ramp, railings were incomplete in that employees could fall more than 4 feet between the end of the ramp guard rail and the barge guard rail.

29 CFR 1917.120(b)(1): Riser height was not 6 to 7.5 inches (15.24 to 19.05 cm), and tread depth was not a minimum of 12 ± 2 inches:    (a) At the load out area the ramp leading to the permanently moored work barge terminated in a step down of approximately 14 inches onto a second step of approximately 2.5 inches in height creating an un-even riser height and tread depth.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap:    (a)  At the barge unloading dock multiple extension cords were coupled together to provide current to the sump pump used to pump water from the down river wing tanks.
