29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) Second floor top plate of lot # 204, 474 Highland Falls Drive, Hiram, GA: Employees installing 7/16" OSB sheathing, joists, trusses and related materials at a height of approximately 11 feet were not provided with a fall protection system.    b) Second floor top plate of lot # 206, 474 Highland Falls Drive, Hiram, GA: Employees installing 7/16" OSB sheathing, joists, trusses and related materials at a height of approximately 11 feet were not provided with a fall protection system

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Front side of the house at Lot # 206, 474 Highland Falls Drive, Hiram, GA- The employer was using an extension ladder to access the top plate of a single family house that was not extended three (3) feet above the landing in that the ladder was approximately six (6) inches below the landing exposing the employees to fall hazards.
