29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:    a) Front of Building - On or about October 16, 2014, the welded frame scaffold used to lay cement blocks did not have mud sills under all legs.  Mud sills were placed under the two end legs on the right end of the scaffold.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:    Front of construction site - On or about October 23, 2014, employees working from a two tier high weld frame scaffold systems did not have safe access to the work platforms.   Scaffold ladders were not provided for the employees.
