OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.    a)  On and before October 28th, 2014 crane operators in the blue slitter bay were permitted to lift 200lbs.  6,000lbs. steel coils with the C style coil grabber which did not have a rated lifting capacity marked on it.      b)  On and before October 28th, 2014 crane operators in the blue slitter bay were permitted to lift 200lbs.  6,000lbs. steel coils with the green inner diameter coil grabber which did not have a rated lifting capacity marked on it.      c)  On and before October 28th, 2014 crane operators in the blue slitter bay were permitted to lift 200lbs.  6,000lbs. steel coils with the red inner diameter coil grabber which did not have a rated lifting capacity marked on it.

29 CFR 1910.179(n)(4)(i): The upper limit switch of each hoist was not tried out under no load, at the beginning of each operator's shift.    a) On and before, 10/28/2014, at the beginning of each shift at the facility located at 355 ledge Rd., Macedonia, OH 44056, the employer did not ensure that crane operators tested the upper limit switch before using the 25 ton Kranco overhead crane.    b) On and before, 10/28/2014, at the beginning of each shift at the facility located at 355 ledge Rd., Macedonia, OH 44056, the employer did not ensure that crane operators tested the upper limit switch before using the 15 ton Zenar overhead crane.    c) On and before, 10/28/2014, at the beginning of each shift at the facility located at 355 ledge Rd., Macedonia, OH 44056, the employer did not ensure that crane operators tested the upper limit switch before using the 25 ton ELY overhead crane.    d) On and before, 10/28/2014, at the beginning of each shift at the facility located at 355 ledge Rd., Macedonia, OH 44056, the employer did not ensure that crane operators tested the upper limit switch before using the30 ton Detroit overhead crane.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):    a) On or about 10/28/2014, at the facility located at 355 Ledge Rd., Macedonia OH., the employer did not ensure that live electrical wires, at the base of the Loopcoo turn style, were properly insulated, covered, or de-energized.    b) On or about 10/28/2014, on the operation deck on the green slitter line, at the facility located at 355 Ledge Rd., Macedonia OH., the employer did not ensure that live electrical wires were de-energized or guarded from protruding out the side and front of a damaged 110 volt electrical outlet box.

On or before 10/28/14, near the #3 blue slitter line at the facility located at 355 Ledge Rd., Macedonia Ohio, the employer did not ensure that the overhead electrical retractable pulley line, which powers the 220 volt coil cart, was protected from damage, pinch points, or other means of exposure damage.
