29 CFR 1926.453(b)(2)(v): The employer did not ensure that a body belt was worn and a lanyard attached to the boom or basket when employees worked from an aerial lift.    This violation occurred on October 28, 2014, where at least one (1) employee utilized an aerial lift without fall protection to gain access to a roof and was exposed to a fall of more than 27 feet to the concrete below.

29 CFR 1926.454(a): The employer did not ensure that each employee who performs work while on a scaffold was trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.     This violation occurred on October 28, 2014, where at least one (1) employee accessed a roof by using an aerial lift without fall protection was exposed to falls of more than 25 feet to the concrete below.
