A continuing and effective extermination program was not instituted  a) Burnet Avenue, union, NJ - Break room (refrigerator):  There were dead roaches observed in and around the break room refrigerator. Health hazards of roaches include but are not limited to allergies, salmonella, pathogenic organisms, disease-causing bacteria and fungi that can be passed on to humans through surface and food contamination.  Condition noted on or about 10/31/2014.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    a) Burnet Avenue, union, NJ site - Warehouse:    Powered industrial vehicle operators were not evaluated at least once every three years. The last documented forklift operator training was 6/22/2011.     Condition noted on or about 10/31/2014.    The Sewing Collection, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 178 (l)(4)(iii), which was contained in OSHA inspection number 315638668, citation number 1, item number 2 and was affirmed as a final order on 12/05/2011, with respect to a workplace located at 1400 West Elizabeth Avenue, Linden, NJ 07306.
