The employer must ensure: 29 CFR 1910.22(a)(3): (3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a. On or about August 13, 2023 and days prior, employees accessed the walking-working surface at the production area where water was leaking from the ceiling drain pipe to the floor surface, exposing employees to slip hazards as a result.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. On or about August 13, 2023 and days prior, strain relief was not provided for the exposed flexible cord which provided 120-volts from the electrical outlet box, exposing employees to an electrical shock hazard as a result.
