OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm including severe burns and explosions from accumulated cotton/polyester dust in and around ignition sources and a disconnected dust collection system. a) Buffing and polishing area: Employees were exposed to fire hazard due to accumulated cotton/polyester dust over 1/32nd of an inch deep on elevated areas of the disconnected dust collection system and accumulations on the floor of up to 30 inches in an area 20 ft. by 5 ft. with a KsT value of 6.07 bar meters per second with live uncovered electrical boxes.

Where wet processes are used, drainage shall be maintained, and false floors, platforms, mats, or other dry standing places should be provided where practicable. a)Buffing and Polishing Area: On or about November 4th, 2014, and prior to, the employer exposed employees to injuries that would result from a fire or explosion from allowing cotton/polyester/calcium carbonate dust to accumulate on floors that would create a fire, explosion or deflagration hazard from failure to adequately conduct housekeeping.
