29 CFR 1910.141(a)(3)(ii): Where wet processes were used, drainage was not maintained and false floors, platforms, mats, or other dry standing places were not provided, where practicable, or waterproof footgear was not provided:  a) Amara Healthcare & Rehab, Augusta, GA - The shower rooms did not have a slip resistant floor or mats to prevent employees from falling, on or about 11/05/14.  a) Amara Healthcare & Rehab, Augusta, GA - The kitchen did not have a slip resistant floor or mats to prevent employees from falling, on or about 11/05/14.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1030(c)(1)(iv): The Exposure Control Plan was not reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure.  Amara Healthcare & Rehab, Augusta, GA  - Employees, such as nurses (RN's LPN's or CNA's) and housekeeping staff, were potentially exposed to blood and/or body fluid while caring for residents at the nursing home. The written exposure control plan was not tailored to the site and was missing elements such, as, but not limited to, (a) exposure determination, (b) housekeeping schedule, (c) personal protective equipment required for each job task, (d) engineering and work practice controls, and (e) appropriate disinfectant, on or about 11/05/14.

29 CFR 1910.1030(d)(3)(i): When there was occupational exposure, the employer did not provide, at no cost to the employee, appropriate personal protective equipment such as, but not limited to, gloves, gowns, laboratory coats, face shields, masks, eye protection, and mouthpieces, resuscitation bags, pocket masks, or other ventilation devices:  a) Employees such as, but not limited to, LPN's, RN's, and CNA's were not provided  "shower boots" to wear to protect their feet from exposure to water, fecal matter and blood that may be present when they are required to shower residents, on or about 11/05/14.
