29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a:  On and before November 6, 2014, the employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that prior to an employee performing servicing such as but not limited to retrieving bowling balls or resetting pins for the AMF Model 82-70; SN. 127058 pin setting machines (32) to protect employees exposed to moving and energized parts.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.      a:  On and before November 6, 2014, on the AMF Model 82-70; SN. 127058 pin setting machine, lane 8, employees retrieving bowling balls or resetting pins were exposed to inadequately guarded horizontal metal shafts are located 3' 4" from the floor, the opening is 3' 2" and the shaft is located 16 1/2"  behind the opening that employees use to access the pin setting machine.    b.  On and before November 6, 2014, employer did not assure that employees working on AMF model 82-70 pin setting equipment on 31 lanes were protected from unguarded horizontal metal shafts inadequately guarded horizontal metal shafts are located 3' 4" from the floor, the openings were 3' 2" and the shafts were located 16 1/2" behind the opening that employees use to access the pin setting machines.

Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section. Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than six feet six inches (6 ft. 6 in.) from the floor or platform may be guarded with a disk covering the spokes.      a: On and before November 6, 2014, located in the rear of the bowling alley, employees retrieving bowling balls and resetting pins were exposed to unguarded pulleys driving the ball return mechanisms for the AMF Model 8270 pin setting machines for each of the 32 bowling lanes.

29 CFR 1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    a: On or before 6 November, 2014 employees were exposed to horizontal belts that were located 56 inches above the floor and did not have guards in place that rose 15 inches above the belt on  AMF Model 82-70 pin setting machines for each of the 32 lanes.
