29 CFR 1910.212(a)(2):  Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  At the worksite located at 17070 Aaron Lane, Belton, Missouri:  Employees were exposed to caught in hazards when working in and around the Brunswick  GS-X pinsetters.  The pinsetters were operating without manufacturer required guarding on the pinsetters to include the elevators and distributors.

29 CFR 1910.303(g)(1)(i):  Sufficient access and working space was not provided for circuit breaker panels to permit ready and safe operation and maintenance.  At the worksite located at 17070 Aaron Lane, Belton, Missouri:  Employees were exposed to electric shock hazards when accessing electrical panels in the pin setting area.  The employer did not maintain adequate working space in front of the panel. 1.  Electrical panels in the pin setting area of The Den were blocked by chairs.   2.  Electrical panels in the pin setting area were blocked by a ladder.

29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  At the worksite located at 17070 Aaron Lane, Belton, Missouri:  Employees were exposed to chemicals such as lane conditioner, isopropyl alcohol, and gloss restorer when cleaning the bowling lanes.  The chemicals may cause eye irritation, skin irritation, and respiratory damage.  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program.
