On November 10, 2014, in the warehouse, the employer did not provide employees who operate the forklift with initial forklift training.    Benson Guam Enterprises was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178 (l)(1) (i), which was contained in OSHA inspection number 907059, citation number 1, item number 4 and was affirmed as a final order on August 16, 2013, with respect to a workplace located at 779 Rt. 8, Hagatna, GU  96910.
