29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Job made work platform: On or about November 4, 2014, the employer allowed employees to be elevated between the ground and roof in a job made, 3-walled, wooden work platform supported by the forks of an all-terrain forklift which exposed employees to falls up to 30 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Roof: On or about November 4, 2014, the employer did not ensure fall protection was provided and/or used which exposed employees to falls of 30 feet to the ground below.
