29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about 11/17/2014, at the above addressed worksite, an employee cutting tile near the edge of a 3rd floor balcony did not have fall protection exposing him to a fall hazard of approximately 30-feet.

29 CFR 1910.1200(e)(1): Employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information will be met.  On or about November 17, 2014, at the above addressed work site, the employer did not develop and maintain a written hazard communication program for employees using tile adhesive.
