29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a.      On November 17, 2014, at bag department, a Packer, packaging plastic bags, was exposed to a noise dose of 72.76%, or an equivalent dBA (decibels A scale) of 87.7 dBA. The noise sampling was conducted for 508 minutes during one work shift. The employer did not administer a continuing and effective hearing conservation program.    b.      On November 17, 2014, at bag department, a Packer, packaging plastic bags, was exposed to a noise dose of 76.37%, or an equivalent dBA (decibels A scale) of 88.0 dBA. The noise sampling was conducted for 493 minutes during one work shift. The employer did not administer a continuing and effective hearing conservation program.      c.      On November 17, 2014, at bag department, a Packer, packaging plastic bags, was exposed to a noise dose of 84.7%, or an equivalent dBA (decibels A scale) of 88.8 dBA. The noise sampling was conducted for 487 minutes during one work shift.  The employer did not administer a continuing and effective hearing conservation program.
