29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a) On or about June 19, 2017: An employee was working inside the Hendrick CM3 2600 saw metal frame guide structure removing a jam. The metal structure is protruding from the floor and employee was exposed to trip and falls and caught in between hazards. b) On or about June 20, 2017: Employees are working next or inside the Hendrick CM3 2600 Horizontal Saw metal frame guide structure, when retrieving plastic sheets and trim to be stacked. The metal structure is protruding from the floor and employees are exposed to trip and falls and caught in between hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about June 19, 2017: The Hendrick CM3 2600 saw was not provided with a guard to protect employees from the point of operation hazards. Since no guarding was provided at the point of operation, allowed the employee to reach inside and pull the plastic to remove the jam; causing an amputation to his right arm. b) On or about June 20, 2017: The Hendrick CM3 2600 saw was not provided with a guard to protect employees from the point of operation hazards. Employees were exposed to amputation hazards.
