29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a.  On or about November 20, 2014 on the west side of a new commercial structure, two employees working for Barrcrete Concrete Services, Inc. They were removing form boards from the structure and were using a scaffold that was not fully planked, exposing the employees to a 46 feet 5 inch fall hazard.

29 CFR 1926.451(g)(1)(v): Each employee on a walkway located within a scaffold was not protected by a guardrail system (with a minimum 200 pound toprail capacity) installed within 9.5 inches (24.1 cm) of and along at least one side of the walkway:  a.  On or about November 20, 2014 on the west side of a new commercial structure, two employees working for Barrcrete Concrete Services, Inc. They were removing form boards from the structure and were using a scaffold that did not have guardrails, exposing employees to a 46 feet 5 inch fall hazard.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stair rail system along each unprotected side or edge:    a.  On or about January 21, 2015 on the south side of a new commercial structure, the employer did not ensure handrails were installed on a stairway, exposing employees to a 3 feet 7 inch (43 inch) fall hazard.
