29 CFR 1926.602(c)(1)(viii)(B): Means were not provided whereby personnel on the platform could shut off power to the truck:  a. On or about May 22, 2019, a subcontracted employee of Harrington Electrical Contractors was using a job made aluminum basket that was not secured to the lift while lifting personnel.  The employee could not shut off power to the lift if needed, exposing employees to struck-by hazards.

29 CFR 1926.451(f)(4): Part(s) of a scaffold that were damaged or weakened, such that the scaffold's strength was less than that required by paragraph (a) of this section, were not immediately repaired, replaced, braced to meet those provisions, nor removed from service until repaired:  a. On or about May 22, 2019, a competent person did not remove from service a job-made aluminum basket being used by subcontracted employees of Harrington Electrical Contractors.  The basket on the left side had the midrail missing , the frame had cracks and the basket was not secured to the lift, exposing employees to struck-by hazards.
