29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about 11/20/14, at 8401 NW 33rd Street Lot 09/Block 06, in a new residential home that was being constructed, employees who were not wearing head protection were observed walking underneath areas where sheathing of a roof took place and were exposed to being struck-by nails, nail gun, and/or wood debris.    b. On or about 11/20/14, at 8401 NW 33rd Street Lot 09/Block 01, in a new residential home that was being constructed, employees who were not wearing head protection were observed walking underneath areas where wood forms were being installed and were exposed to being struck-by tools, wood debris, and/or nail guns.   c. On or about 11/20/14, at 8401 NW 33rd Street Lot 18/Block 04, in a new residential home that was being constructed, employees who were not wearing head protection were observed walking underneath areas where wood forms were being installed and were exposed to being struck-by tools, wood debris, and/or nail guns.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 11/20/14, at 8401 NW 33rd Street Lot 09/Block 06, an employee using a nail gun while installing sheathing on roof trusses without any means of eye protection was exposed to being struck-by a nail in the eye.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. On or about 11/20/14, at 8401 NW 33rd Street Lot 09/Block 06, employees installing wood sheathing on roof trusses of a new residential home in construction and not using a means of fall protection were exposed to a 24 foot fall hazard.  b. On or about 11/20/14, at 8401 NW 33rd Street Lot 09/Block 01, an employee standing on a tie-beam of a new residential home in construction and not using a means of fall protection was exposed to an 11 foot fall hazard.  c. On or about 11/20/14, at 8401 NW 33rd Street Lot 18/Block 04, employees who were tying rebars on top of a new residential home in construction and not using a means of fall protection were exposed to an 11 foot fall hazard.  According to STD 03-11-002 Compliance Guidance for Residential Construction 1926.501(b)(1) standard applies based on the material used in framework of the structure.   PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
