29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches. M/V Oscar Dyson: On or about November 18, 2014 and times prior thereto, there was an open flush hatch open where employees would pass by. This exposed them to a fall hazard of 8 feet to the deck below.

29 CFR 1915.56(b)(4): When welding cable became worn to the extent of exposing bare conductors, the portion exposed was not protected by means of rubber tape, friction tape, or other equivalent insulation. Oscar Dyson weather deck: On or about November 18 2014 and times prior thereto, a welding cable was cut to the conductor. This exposed employees to potential burn/shock hazard if they contacted the conductor during welding operations.
