29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, on the 10:12 pitch roof, employees, attaching slate shingles to the north corner of the roof, were not protected from a fall hazard of 20.4 feet by the use of a guardrail system, safety net system, personal fall arrest system, or alternative fall protection method.   b)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, on the 10:12 pitch roof, an employee, moving along the northwest side of the slate shingled roof, was not protected from a fall hazard of 20.4 feet by the use of a guardrail system, safety net system, personal fall arrest system, or alternative fall protection method.    No abatement certification or documentation required.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  a)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, on the 10:12 pitch roof, employees, accessing and egressing the southwest edge of the roof, were not protected from a fall hazard of 21.7 feet in that they were using the round-crossbarred lift hoist track as a ladder because no ladder was provided for their use.   b)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, employees, descending the ladder were unable to easily climb off of the bottom of the round-crossbarred lift hoist track because the lift hoist platform was in position at the bottom of the track, blocking access to the bottom 2.5 feet of the track.   No abatement certification or documentation required.

29 CFR 1926.1053(b)(21): Each employee was not grasping the ladder with at least one hand when progressing up and/or down the ladder:  a)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, an employee, carrying rope, while ascending the round-crossbarred lift hoist track, was unable to maintain a grasp of the ladder with at least one hand.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of a ladder was not kept clear:  a)  Girard Avenue & Wyomissing Boulevard, Wyomissing, PA - On or about November 25, 2014, employees, descending the ladder were unable to easily climb off of the bottom of the round-crossbarred lift hoist track because the lift hoist platform was in position at the bottom of the track, blocking access to the bottom 2.5 feet of the track, and the area around the base of the lift hoist was not kept clear with stacks of slate roofing material, a tarp, and a bowl all within close proximity of the bottom of the lift hoist track.   No abatement certification or documentation required.
