Suite 1, Hermitage, PA 16148-Fabrication area south wall next to oxygen cutting area, on or about December 11, 2014 and times prior thereto:  An oxygen manifold system was directly adjacent to an emergency exit door.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1, which is incorporated by reference as specified in CFR 1910.6, Section 5.8.4, all cylinders were not secured to prevent falling or rolling:    a.)    Extreme Machine & Fabrication, Inc., 2340 Quality Ln. Suite 1, Hermitage, PA 16148-Fabrication area south wall, on or about December 11, 2014 and times prior thereto: A compressed gas cylinder of propane was free standing and was not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over.

29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:       a.)    Extreme Machine & Fabrication, Inc., 2340 Quality Ln. Suite 1, Hermitage, PA 16148-Microwave (General Electric brand "Spacemaker II") next to oxygen cutting area, Fabrication area south wall, on or about December 11, 2014, and times prior thereto:  Wipe Samples were obtained from a microwave next to the oxygen cutting area in the fabrication area of the facility.  Analytical results indicated the presence of toxic metals, as follows:       1.)    Microwave (General Electric brand "Spacemaker II")next to oxygen cutting area (Fabrication area-South wall):  10.0770 ug Manganese, 491.3600 ug Iron Oxide;       b.)    Extreme Machine & Fabrication, Inc., 2340 Quality Ln. Suite 1, Hermitage, PA 16148-Microwave(Whirlpool brand) next to Fabrication office area, Fabrication area south wall, on or about December 11, 2014, and times prior thereto:  Wipe Samples were obtained from a microwave next to the fabrication offices in the fabrication area of the facility.  Analytical results indicated the presence of toxic metals, as follows:       1.)    Microwave (Whirlpool brand) next to fabrication offices (Fabrication area-South wall):  2.9172 ug Copper, 15.5480 ug Manganese, 36.5360 ug Cobalt, 834.6000 ug Iron Oxide, 23.8420 ug Chromium.
