The top of the furnace's guardrail system was not equipped with a mid-rail or equivalent member.  Employees were accessing the top of the furnace to perform operations that included but were not limited to, attaching the crane to the furnace lid to remove it and/or place it on the furnace.

In order to access the bottom rung of the fixed ladder, employees had utilized a step stool to navigate a break in elevation of 38 inches from the floor to the bottom rung of the ladder.  Employees were potentially exposed to a fall hazard from the step stool kicking out when accessing/egressing the bottom rung of the ladder.
