29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck.    (a) Location - Loading dock:  On or about 11/29/14, an employee was operating a Crown Stand-up Rider Lift Truck without being trained.      Greencore USA, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 910554, citation number 1, item number 3 and was affirmed as a final order on September 16, 2013, with respect to a workplace located at 2 Opportunity Way, Newburyport, MA.
