Hinds Bock Machine, Filler/Depositor (MN 12P-04, SN 7337).  On or about May 11, 2017 to May 18, 2017, machine operators performed servicing and maintenance such as clearing jams and were exposed to hazards likely to cause amputation when working near the moving parts of the rotary cylinder/manifold. Employees were not provided with adequate energy control procedures that contained a specific statement of the intended use of the procedure when clearing jams, and specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    1) turn off and shut down the machine,  2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,  4) verify that isolation and deenergization of the machines or equipment have been accomplished.    Primary Room, CP02, Hinds Bock Machine, Filler/Depositor (MN 12P-04, SN7337).  On or about May 11, 2017 to May 18, 2017, machine operators who performed servicing and maintenance tasks, including clearing jams, were exposed to the moving parts of the rotary cylinder/manifold that supplied the dispensing tubes with food product.  Operators were not required to control hazardous energy sources in the required sequence.  Employees were exposed hazards likely to cause amputation and laceration from moving parts where the energy source was not isolated.  Alternate protection, i.e. machine guarding, was not adequate.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
