29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle. Location: Production Floor Employees are exposed to amputation hazard while operating the shrink wrap machine where the intake opening was not adequately guarded and employee amputated two fingers when he reached into the machine to push trays through and the machine cycled.
