29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew: Mi Mama's Tortillas 828 S 17th Street, Omaha, NE 68108: The employer is failing to ensure that employees performing maintenance or service to equipment were afforded a level of protection equivalent to a personal lockout device. Employees of Mi Mama's Tortillas are required to perform cleaning activities at least once per week. Maintenance is responsible for de-energizing the equipment and authorized employees place a lock on one of the devices used to de-energize. The authorized employees only are issued one lock and tag and therefore are unable to affectively de-energize the machinery prior performing cleaning.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: In the Maintenance Shop on the BENCO Mill (UNK serial#): the employer is failing to ensure that unused openings on the electrical supply box are effectively closed. During the inspection 2- ½ knockouts were missing from the electrical box on the back side of the mill.
