29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems:    On or about December 3, 2014, employer did not ensure that employee(s) working on the roof were provided with and required to use adequate fall protection. Employees were exposed to fall hazards from and elevation exceeding 6 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       On or about December 3, 2014, employed did not ensure the ladder that was being used to access the residence roof was extending at least 3 feet from the upper landing. This condition exposed employees to falls exceeding 6 feet.
