29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  Worksite at 716 Forest Circle, South Charleston, WV:  Employees were installing the ridge vent and operating a leaf blower on a newly installed 5:12 roof and were not protected from falling approximately seven to twelve feet by use of a personal fall protection system, guardrail system, safety net system, or any other approved alternative method as observed on December 8, 2014.  ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  (a) Job site at 716 Forest Circle in South Charleston, WV: The employer did not have a person in the company competent in OSHA construction regulations in that the foreman sent his employees onto the roof of a house to finish installing a ridge vent and operate a leaf blower with fall protection harnesses on but no anchor points and tie off lines to attach to as observed on December 8, 2014.  (b) Job site at 716 Forest Circle in South Charleston, WV: The employer did not have a person in the company competent in OSHA construction regulations in that the foreman sent his employees onto the roof of a house using a ladder that extended less than three feet above the roof edge as observed on December 8, 2014.  ABATEMENT CERTIFICATION REQUIRED
