29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose:  On or about December 09, 2014, the employees were using several extension cords which they had connected together in order to make longer extension cords so that they could reach their designated outlets. Employees were exposed to electrical hazards.

Employees were exposed to electrical shock hazards.

Employees were exposed to electrical shock hazards.
