29 CFR 1910.145(f)(3): Safety instruction signs were not used where there was a need for general instructions and suggestions relative to safety measures:    (a)  WVDNR Pad 6, Jacksonburg, WV:  The Entech Dust Collector, with an inclined pipe containing an auger, did not have a hazard warning sign located at the bottom end of the auger pipe to warn employees of the moving auger, as determined on or about December 12, 2014. 29 CFR 1910.151(b): There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:    (a)  WVDNR Pad 6, Jacksonburg, WV:  On or about December 12, 2014 employees worked on a site that was located in a remote area that was not close to a medical facility and at least one employee of KSW Environmental LLC that was trained to render first aid was not on site at all times or in close proximity to render first aid.      ABATEMENT CERTIFICATION REQUIRED
