29 CFR 1910.213(c)(3): Each hand-fed circular ripsaw shall be provided with nonkickback fingers or dogs so located as to oppose the thrust or tendency of the saw to pick up the material or to throw it back toward the operator. They shall be designed to provide adequate holding power for all the thicknesses of materials being cut.  a.  At the workplace located at 2447 US 62, Winesburg, Ohio, in the bed department, the employer did not provide the C-25 plywood rip saw with nonkickback fingers or dogs, thereby exposing employees to a struck-by and amputation hazard.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a.  At the workplace located at 2447 US 62, Winesburg, Ohio, in the assembly department, the glue clamp machine, The Taylor, model 800136, had a chain and sprocket four and half feet from the floor that was not guarded, thereby exposing employees to a caught-in and amputation hazard.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Yutzy's Woodworking in Winesburg, Ohio:  a.  On the 2014 OSHA 300 Log the employer did not provide all information required in Column F such as the actual cause of injury and actual injury.       1. Case 20, column F stated, "Fell unloading shoulder back", which is insufficient to determine the actual injury.        2. Case 22, column F stated, "Amputation Lft index finger", which is insufficient to determine what caused the injury.
