29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and regulatory requirements for controlling and eliminating hazards in the workplace: a) Employees were not provided sufficient instruction during window replacement operations, to ensure window pivot posts will not be cut ahead of time, before the mast climber scaffold is in position outside of the window. A window fell from the 17th floor exposing employees to falling object hazard. On or about 12/18/2014 Location: 163 W 125th Street, New York, N.Y.
