Preventive maintenance was not conducted on the hoist braking system. The hoist brake has damaged threads which prevented taking the brake completely apart.   2. Preventive maintenance was not conducted on wireless control pendent knob. The start knob was broken in which the employee were utilizing a screw driver to start the crane with the wireless control pendent.
