29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) South side, new residential construction located at 11760 Stratham Drive, Alpharetta, GA 30009: Employees operating Hitachi (Model NT65M2S) 2-1/2-inch Finish Nailers and Bostitch Coil Siding Nail Guns at 120 PSI were exposed to eye injuries when protective eyewear was not provided while operating these tools.

29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:    (a) South side, new residential construction located at 11760 Stratham Drive, Alpharetta, GA 30009: Employee was exposed to amputation hazards while operating two Makita (Model 5007F) 7-1/4-inch Circular Saws with no lower guard in place as a wood shim was used to prevent the lower guard from returning below the base plate after a cut was made.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    a) South side, new residential construction located at 11760 Stratham Drive, Alpharetta, GA 30009: Employees were exposed to falls of approximately 17-feet, 5-inches to the lower level while installing Hardie-Plank Siding from a pump jack scaffold with no fall protection system in place.
