29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300) and for the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment:  55 Impulse Industrial Drive, Dawsonville, GA (Impulse Mfg.) - The employer is not maintaining the OSHA 300 logs and/or any equivalent forms for the workplace injury which occurred for the annual year of 2014; the following was not recorded onto the OSHA-300 logs, on or about January 13, 2015.  a. On or about November 9, 2014,  - Welding Fabricator - an employee was struck and cut on the face by a portable hand-held grinder.  This injury resulted in the employee receiving sutures to the eye and prescription medication from the physician.
