29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:     The employer does not provide each employee engaged in steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet above a lower level with fall protection.  This violation was observed on or about January 21, 2015, at the job site where employees were installing steel beams at a height of 23 feet without being protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling to lower levels when engaged in steel erection activities more than 15 feet above a lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer does not ensure that the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access.  This violation was observed on or about January 21, 2015, at the job site where employees were using portable ladders to gain access to working surfaces without the ladder side rails extending at least 3 feet past the upper landing surface.       Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the ladder side rails extend at least 3 feet above the upper landing surface to which the ladder is used to gain access
