29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards:  a.  690 Camp Glisson Road, Dahlonega, GA (Glisson Camp & Retreat Center) - An employee was exposed to a fall hazard.  The employee who was working from a mobile scissor lift was not trained in the recognition of hazards associated with the type of scissor lift being used (Skyjack), on or about January 16, 2015.

29 CFR 1904.39(a): The employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident:    a. 690 Camp Glisson Road, Dahlonega, GA (Glisson Camp & Retreat Center) - The employer failed to notify the Atlanta-East OSHA office of an in-patient hospitalization within 24 hours of being admitted.  The employee was admitted as an in-patient on January 16, 2015 and was not reported to the OSHA Area Office until January 20, 2015.
