29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:    a)  At the jobsite, on the North Side of the building, on or about 1/23/15:  Employees were working on a ladder jack scaffold, without adequate access to the work surface, without fall protection, and without having been erected under the supervision of a competent person.      Abatement certification must be submitted for this item.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally and/or 24 inches (61 cm) vertically from the other surface:  a)  At the jobsite, on the North Side of the building, on or about 1/23/15:  Employees were working on a ladder jack scaffold, where the pick was approximately 16 feet from grade, and where workers had to climb over the pick to access the work surface.
