29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,      Location: Door 487; BOF pit    On or about 22 January, 2015 ESM Group Inc., and MPW Industrial Services, Inc., employees were exposed to a fall from an open sided floor of over 20 feet into a pit that was not guarded by a standard railing, as parts of the railing were constructed from plastic safety tape.

29 CFR 1910.23(e)(5)(iv): The mounting of handrails shall be such that the completed structure is capable of withstanding a load of at least 200 pounds applied in any direction at any point on the rail.   Location: Door 487; BOF pit  On or about 22 January, 2015 ESM Group, Inc., and MPW Industrial Service, Inc., employees were exposed to a fall from an open sided floor of over 20 feet into a pit that was not guarded by a standard railing on the sides that were capable of withstanding a load of at least 200 pounds, as parts of the railing were constructed from plastic safety tape.
