On January 30, 2015, on the east side first level interior of the house, there was no guard on a 110 VAC Bosch 1380 Slim right angle grinder that employees that used to cutout trenches in the concrete floor for electrical conduit.

On February 3, 2015, On the west side roof of the house, there was no fall protection for employees that stood on a 22-foot high by 5-foot wide by 42-foot long overhang while troweling concrete onto a railing.

On February 3, 2015, on the west side exterior of the house, there were no stairrails along the unprotected sides or edges on the stairway with 10 risers to a 6.5 foot high landing or on the stairway with 8 risers from the landing to the 11.25 foot high second level.
